PLEASE READ -- IMPORTANT: Due to HIPAA regulations, forms cannot be faxed. All forms must be handed in to the high school nurse. FAXED FORMS WILL NOT BE ACCEPTED-- ORIGINALS ONLY.

*ALL PHYSICALS COMPLETED BY A PRIVATE PHYSICIAN MUST BE APPROVED BY THE SCHOOL DOCTOR PRIOR TO THE STUDENT ATHLETE PARTICIPATING IN PRACTICE/TRYOUT.

Electronic Travel Release Form

IMPORTANT: This form must be received by the Athletic Office 24 hours in advance of the date of the sporting event requested on the Travel Release Form. This form must be approved by the Athletic Director. Forms sent after that time will not be approved.
