POINT PLEASANT HIGH SCHOOL ATHLETIC DEPARTMENT
DIRECTIONS FOR SUBMITTING ONLINE SPORTS REGISTRATION
All students who intend to participate in sports must register through the ArbiterSports Registration website (formerly FamilyID) for each sport season. Each student athlete registration includes his/her State of New Jersey Department of Education Health History Update Questionnaire which must be completed each season.
Click here for the HS ArbiterSports Registration website and follow these instructions to register for sports: - Click on correct season to register. (If you already have an ArbiterSports account, you can click “log In” to log into your account).
- Set up a ArbiterSports account by entering your family name, email address and password.
- You will receive an email with a link to confirm your new account. (If you do not see a email, check your spam or junk folders).
- Click on the link to confirm your email. This will return to you the registration page.
- Fill out the information requested on the form and submit.
- After your sign-up is complete, you will receive an email from ArbiterSports confirming your sign-up was successful.

FOR THE HEALTH OFFICE:
All paperwork must be turned in as a complete package to the School Nurse NO LATER THAN NOTED DEADLINE. Any missing information will result in the delay of your child's medical clearance; all paperwork will be returned to the parent for proper completion.
**ALL PHYSICALS COMPLETED BY A PRIVATE PHYSICIAN MUST BE SENT TO & APPROVED BY THE SCHOOL DOCTOR PRIOR TO THE STUDENT-ATHLETE PARTICIPATING IN PRACTICE / TRYOUT.**
Pre-participation Physical Evaluation - HISTORY FORM & PHYSICAL EXAM packet (4 pages)
Required once per year, to reflect a medical examination within 365 days prior to the first day → Physical Packet
► HISTORY FORM — Completed, signed and dated by parent and athlete. Please make sure that the physician records the date of exam on line 1.
► ATHLETES WITH SPECIAL NEEDS: SUPPLEMENTAL HISTORY FORM — when applicable, should be completed. If not applicable, please sign, date and write N/A.
► PHYSICAL EXAM FORM must be completed in its entirety — please verify that nothing is left blank before leaving the physician's office!
► CLEARANCE FORM — must be complete in its entirety — Physician must stamp, sign and date exam portion AND must sign and date the Completed Cardiac Assessment Professional Development Module near the bottom of the page.

MAKE SURE YOU HAVE A PHYSICAL PACKET AND BRING IT WITH YOU TO THE PHYSICAL. THE HEALTH HISTORY FORM MUST BE FILLED OUT PRIOR TO THE PHYSICAL. ALL FORMS MUST BE SIGNED. IF ANY PAPERWORK IS INCOMPLETE, THE PHYSICAL WILL NOT BE ACCEPTED.
