DIRECTIONS FOR SUBMITTING FORMS ON-LINE
We are excited to announce that we are now offering the convenience of online registration through our partner company, FamilyID. Other required forms, as well as, athletic registration must be completed each season through the online website FamilyID at www.familyid.com/point-pleasant-borough-high-school. This website will help the athletic department be more administratively efficient and environmentally responsible.  Once on the FamilyID website, please follow these instructions to register for winter sports:
  • Click on 2014-2015 Winter Athletic Registration or Individual. (If you already have a FamilyID account, you can click “log In” to log into your account).
  • Set up a FamilyID account by entering your family name, email address and password.
  • You will receive an email with a link to confirm your new account. (If you do not see a email, check your spam or junk folders).
  • Click on the link to confirm your email.  This will return to you the 2014-15 Winter Athletic registration page.
  • Fill out the information requested on the form and submit.
  • After your sign-up is complete, you will receive an email from FamilyID confirming your sign-up was successful.
 
IMPORTANT PHYSICAL INFORMATION
 

PLEASE REVIEW -  IMPORTANT INFORMATION REGARDING NEW PHYSICAL FORMS
Dear Parents and Guardians of High School Athletes,
          The purpose of this letter is to inform all Point Pleasant Borough High School parents/guardians of the new Pre-participation Physical Evaluation (PPE) forms and Health History Update Questionnaire form, mandated by  the New Jersey Department of Education that all school districts implement as of May 1, 2014 (as per P.L. 2013, c. 71, the Scholastic Student-Athlete Safety Act).  Effective immediately, these forms must be used for all students that are interested in participating in school-sponsored interscholastic athletics.
          Please note,  one major change is the new Clearance Form of the PPE.  This form requires a signature and a physician’s stamp from your medical home physician, APN, PA and/or school physician that attests to the completion of the Student-Athlete Cardiac Assessment module.  It is important that parents/guardians understand that a physical will not be accepted by the school district without the completion of ALL PPE forms, including the completed Clearance Form.  Also note, the Health History Update Questionnaire form cannot be dated 90 days prior to the start of the season.  As in the past, a physical must be dated within one (1) year from the time of the first practice date.
          Every physical conducted by a medical home physician, APN, or a PA MUST be reviewed and signed by the school physician prior to your child participating in their sport.  Therefore, it is imperative that all physicals are completed and submitted to the school nurse’s office by August 4th for your son/daughter to be eligible to participate on the official start date of the fall season.
          Along with the new PPE forms, it is required that parents/guardians and student athletes read and sign the following forms: Sudden Cardiac Death in Young Athletes Pamphlet, Parent Consent form, Student Athlete Contract, NJSIAA Steroid Testing Policy, and Sports-Related Concussion and Head Injury Fact Sheet and Parent/Guardian Acknowledgement Form. 
          These forms must be completed through the online website FamilyID at www.familyid.com/point-pleasant-borough-high-school.  This website will help the athletic department be more administratively efficient and environmentally responsible.  On the back of this letter you will find the directions for completing all the necessary forms through the FamilyID website.

Please click on the Forms link on the left hand side of the screen to access all of the forms for the Athletic Department.  These forms can be printed out.  Physical forms cannot be sent by fax or on-line.
